Any pet under six months or with medical conditions that we place in foster will need to have regular vet appointments at our clinic. Generally, these appointments will be scheduled Monday-Thursday between 1-3:30 pm, and Friday-Saturday between 10am-3pm. I acknowledge that as a foster it is my responsibility to bring my foster pet in for these appointments as scheduled or reach out to the foster Assistant Manager to reschedule as needed. * Initial*

Foster Program Waiver Release

If approved to foster, I understand:

Pet foster parents are expected to bring their pets in for regular check ups at our clinic. Puppies and kittens will require appointments for vaccine boosters every two weeks. Other pets may require more frequent appointments based on medical needs.

Pet fosters will be given a sheet with appointment times and dates when they pick up their pet. When they need to reschedule, fosters will contact the foster coordinator as soon as possible.

Fosters will keep cats in their care indoors at ALL TIMES.

Fosters will not allow puppies or dogs off leash unless they are in their own, fenced-in yard. Dogs and puppies will not be off leash in parks, even dog parks.

Fosters will give any necessary medications to their pets as prescribed by the HSMSC clinic.

Foster placements are at the discretion of HSMSC and can be ended at any time, for any reason, by HSMSC or the foster parent.

Foster Program Waiver Release

Thank you for your interest in HSMSC’s Crisis Foster Program. This is a unique program that allows volunteers to take shelter dogs and cats to their homes for short-term fostering. By participating in the Foster Program, I agree to the following:

I understand that pets placed through the Crisis Foster Program are owned pets temporarily housed due to a verified emergency. The purpose of this placement is reunification with the pet’s legal guardian.*

I acknowledge that this placement is temporary and that the pet remains the legal property of the original guardian. No ownership rights are transferred to the foster caregiver.*

I understand crisis foster placements operate on a month-to-month basis and may last up to 90 days unless otherwise approved in writing by Paw Shield.*

I agree to provide safe housing, food, water, and daily care. I agree to follow all Paw Shield policies and instructions and to notify Paw Shield immediately of illness, injury, behavioral concerns, or emergencies. I shall not transfer the pet to another individual without written approval from Paw Shield.*

I understand that ALL non-emergency veterinary care must be pre-approved by Paw Shield. Emergency veterinary care must be reported to Paw Shield as soon as reasonably possible.*

I understand that all communication regarding the pet must be coordinated through Paw Shield. I shall not independently contact the guardian unless authorized and agree not to share personal information about the guardian.*

I understand that Paw Shield cannot guarantee the health, temperament, or behavior of any pet placed in foster care, and I agree to foster at my own discretion.*

I understand that Paw Shield reserves the right to remove the pet from foster placement at any time if deemed necessary.*

Acknowledgement and Signature

Under penalty of law, I certify that my answers are true and complete to the best of my knowledge. I am aware of the common obedience problems with shelter pets and I agree to allow my adopted/fostered pet time to adjust to its new environment. If my living circumstances change and I am unable to provide suitable accommodations for my adopted/fostered pet, I understand that I am to surrender it back to the Humane Society of Memphis & Shelby County.*
